Urinary tract infections in pregnancy

Urinary tract infections (UTI) associated with pregnancy represents the important problem of obstetrics, urology and nephrology, caused by changes in clinical features UTI, approaches for assessment and treatment UTI and high risk of obstetrical, urologic and neonatal complications in pregnant women with UTI. This review reflects epidemiology, etiology and definition of physiological changes of urinary tract in pregnancy. Characteristics of antibacterial drug management and treatment of different clinical forms of UTI will be described in second review.

Dynamics of changes of a spectrum of medicinal fastness m.tuberculosis, discharged from the patients who are taking place on treatment in the hospital ipp (1990 to 1999)

On the data of laboratory of microbiology ІPP (Institute of pulmonology and phthysiatry) the retrospective analysis of a drug resistance 8426 strains M. tuberculosis, discharged from the patients who are taking place on treatment in the hospital with 1990 for 1999 was carried out. The augmentation of strains with a specific gravity resistance to drugs is marked. The frequency of abjection of multiresistance variants M. tuberculosis has increased by 24,4%. The level of polyresistance strains of M. tuberculosis in frame of a resistance is stable. The parameters of a monoresistance have decreased on 13,4%. In according with deterioration of the epidemiological situation in Ukraine the augmentation of frequency of abjection of strains M. tuberculosis, resistance to 5 and 6 drugs is scored with which treat a tuberculosis. By 1999 the frequency of abjection of variants M. tuberculosis, refractory to 6 drugs (Isoniazidum, Rifampicinum, Streptomycinum, Kanamycinum, Ethambutolum and Etioniamidum) has increased up to 11,7% and made 1/6 part of all abjected strains of M. tuberculosis with drug resistance.

Studies of bacteriostatic blood activity in different regimens of polychemotherapy of patients with the destructive pulmonary tuberculosis.

The aim of the present work was to study the bacteriostatic blood activity (BBA) in various regimens of polychemotherapy. We analyzed BBA in 152 patients with destructive pulmonary tuberculosis (in those mainly newly diagnosed) by 3 various regimens of polychemotherapy. Maximum value of BBA in regimen 1, with 5-6 basic antituberculous drugs in a daily usage of 5 drugs, reached high level (bacteriostasis was seen in the blood dilutions in 32-512 times) in 100% of patients. BBA was high in 91,8% of cases with regimen including 4 drugs every day (regimen 2). In the regimen 3 including 5-6 drugs all or part of which were used intermittently (ever other day) and the rest of them every day, i.e.3-4 drugs per day, the maximum level of BBA was high at 91,5% of cases. The highest level of BBA (from 1:256 to 1:512) was marked with regimen 1 in 64,3% of cases, with regimens 2 and 3 - only at 14,6% examined patients. At the same time only high or high and average rates of BBA (1:512-1:8) during all 24 hours were marked in 98,2% of cases with regimen 1, in 51,0-61,7% of patients with regimens 2 and 3. Abacillation was reached at 100% of the patients with the first regimen and 97,9% with the second and third regimens, accordingly in 1,51*0,11; 1,90-1,91*0,14 months. BBA maximal levels and levels during 24 hours with regimen 1 were higher than these levels with regimens 2 and 3, for this reason the abacillation terms were short at the first regimen.

The triple therapy: pantoprazole, amoxycillin and azithromycin - is effective in eradication of helicobacter pylori infection

In the study the efficacy and safe of therapeutic regime containing of Pantoprazole (P), Amoxycillin (A) and Azitromycin (Az) (7 days) for Helicobacter pylori (HP) eradication in duodenal ulcer patients were investigated. The control group patients with HР-associated duodenal ulcer disease received 7-day - standard triple therapy. HР eradication was assessed by 13C-UBT. The eradication rate was 94% in P+A+Az (1000 mg/day for 3 days) group and 73,9% in P+A+metronidazol. Triple therapy with P+A+Az (1000 mg/d for 3 days) for 7 days is effective and safe and enable to achieve an acceptable HP eradication rate in duodenal ulcer patients.

The link between helicobacter pylori with ischemic heart disease

It has bean studet evaluate a possible role of H. pylori in development of ischemic heart disease (IHD). We enrolled 62 pts with IHD. The specific IgM and IgG antibodies and presence of H. pylori by polymerase chain reaction in atherosclerosis plaques of the coronary arteries and aorta specimens of 50 autopsy cases were aims of the study. The study showed that H. pylori infection is frequent in the patients with IHD. H. pylori is found in the atherosclerotic plaques of the coronary arteries and aorta of the each tenth patients who had died because of myocardial infarction. The study has indicated that H. pylori infection contributes to both the development and progression of coronary atherosclerosis.

Efficacy evaluation of ciprofloxacin in the treatment of patients suffered from acute pyelonephritis

Pharmacoeconomic analysis of antibacterial therapy in patient with non-severe community-acquired pneumonia

The results of studies included 104 patients with community-acquired pneumonia has shown that appliance benzylpenicillin-sodium (500 mg i/m 6 times-a-day), midecamycine (400 mg thrice-a-day), azitromycine (500 mg once-a-day), amoxycilline + clavulanat (625 mg thrice-a-day) are equal high efficacy. The antibacterial therapy safety estimation has not showed differences on frequency of side effects.

Research of antitumoral activity and toxicity of doxorubicine conjugated with transferrine and albumin on experimental animals

The purpose of research was study on experimental animals of antitumoral activity and toxicity of acid-sensitive conjugates of doxorubicine with transferrin and albumin in comparison with free drugs. Original results concerning analysis of toxicity and antitumor activity of different doses of conjugates as potential remedy for chemotherapy of tumors are presented. It was established that using of these conjugates allows essentially reduce a toxicity of chemotherapy when antitumor influence retains or even raises.

Antimycobacterial activity of ciprofloxacin and lomefloxacin relative culture h37rv

Adjuvant and neoadjuvant chemotherapy in the treatment for patients with lung cancer

We estimated the results of radical surgery for patients with lung cancer using adjuvant (129) and neoadjuvant chemo- (45) and thermochemotherapy (62). Neoadjuvant thermochemotherapy was more effective and prolonged 3-year survival time, especially in stage ІІІА lung cancer patients.

Frequency and dynamics of isolation of l-forms of m. Tuberculosis in the patients with relapses of lung tuberculosis

64 patients with relapses of lung tuberculosis were examined. L-forms of M. tuberculosis were detected in 26 (40,6%) patients. In 73,1% of all cases L-forms turned out to be instable and reversed after 1 to 3 passages in vitro into typical bacterial form of the pathogene. Revertant cultures had the same characteristics as the typical bacteria, isolated in the same patients. L-forms excreters should be regarded as a group that needs follow-ap and antibacterial therapy.

The influence of new ukrainian multicomponent herbal extract on oxidative homeostasis in rat's viscera and blood at experimental cisplatin intoxication

The influence of new Ukrainian multicomponent herbal extract (MHE) - drug Polyphytol-1 on the data of oxidative homeostasis in rats with experimental cisplatin intoxication was studied. It was established that MHE substantially prevents cisplatin caused lipid peroxidation processes in liver, myocardium, brain, spleen and blood and also prevents disturbances in glutathione antioxidant system in liver, myocardium, brain, spleen and intestine. This plant-origin drug may be recommended for prevention of toxic effects of platinum antitumor drugs.

Retrospective pharmacoepidemiological survey of antibiotic therapy of community acquired pneumonia

In order to investigate the spectrum and rationality of antibiotic therapy of community acquired pneumonia before wide-spread discussion of international recommendations for treatment of pneumonia, a retrospective investigation of 225 medical records of patients with diagnosis of community acquired pneumonia treated in the City Hospital № 1 from January to December 1998, was carried out. It was found out that the following were the preparations of the first choice and were prescribed most frequently: fluoroquinolones (12%), cephalosporins of the first generation (11,1%), macrolides (10,2%), semisynthetic penicillins (6,7%). 38% of patients underwent monoantibiotic therapy, 10,2% of patients underwent sequential scheme of medicine application. In most cases the received data witness about a possibility or empirical character of medicine choice, and it allows to assume, that the choice of antibiotic will be more effective by taking into consideration the antibiotic spectrum and strict evaluation of the initial response to the antibiotic therapy.

Efficiency of the chemotherapy of ovarian cancer depending on regimens and doses of cytostatics

With the purpose of evaluation the efficiency of the use of the regimens in chemotherapy with preparations of platinum (СР and САР combinations) depending on the dose and modes of the administration of the cytostatics, 378 patients with ovarian cancer 1-4 stage were examined. All the patients had the ovarian tumors of the epithelial morphological structure with the prevalence of serous (76%) and non-differentiated (9%) forms. The age of patients ranged from 29 to 73. The doses of cytostatics were standard and corresponded to the mode of application (3 or 4 weeks). It has been established, that the duration of non-relapsing period depends on the numbers of chemotherapeutic courses of treatment taken , combinations of anticancer agents used and their doses' "intensity". At the 1-st and 2-nd stages of ovarian cancer it's more efficient to use 5 courses of chemotherapy with a four-week interval or six courses with a three-week interval. In the patient at the third stage of ovarian cancer it is more efficient to use either 8 courses with a three-week regimen or 6 courses with a four-week interval of cytostatics' injections. With using any of these two regimens of chemotherapy an equal dose intensity of agents were attained during 6 to 6,5 months, this fact enables to acquire the better results of the therapy in the ovarian cancer patients.

Efficiency of amyzon in complex treatment of the patients with erysipelas

The clinical efficiency of a new domestic pharmaceutical AMYZON in the erysipelas complex therapy has been proved. AMYZON chemotherapy accelerates liquidation of the syndrome of the infectious toxicosis and regression of the locus of the erysipilatous inflammation, and also reduce frequency of purulent-inflammatory complications. The immunomodulating effects of the drug has been revealed. The received data allows to recommend application of AMYZON for chemotherapy of the erypsipelas patients.
