CLARITHROMYCIN IN TREATMENT OF LOW RESPIRATORY TRACT INFECTIONS

The data regarding the efficacy of clarithromycin in treatment of low respiratory tract infections and worldwide prevalence of antibiotic resistant strains of the causative pathogens have been presented. It was proved clarithromycin nowadays to be an antibiotic of choice for treatment of low respiratory tract infections.

THE TREATMENT OF PATIENTS WITH NOSOCOMIAL PNEUMONIA

The issues of nosocomial pneumonia treatment are discussed in the article. Antibiotics of differrent groups were analyzed concerning their rational combinations and demands for prescription. Empirical methods of initial antibiotics therapy were offered. A special attention has been focused on new and problematic aspects of nosocomial pneumonia treatment.

A CHOICE OF EMPIRIC ANTIBACTERIAL THERAPY FOR TREATMENT OF PATIENTS WITH VENTILATOR-ASSOCIATED NOSOCOMIAL PNEUMONIA

The data on possibility and feasibility of meropenem administration as empiric first_line therapy in patients with ventilator_associated nosocomial pneumonia, especially in severe cases, or in settings with high prevalence of 3rd_4th generation cefalosporins, ciprofloxacin and antipseudomonad penicillins resistant strains, have been presented.

MODERN REGIMENS OF ANTIMYCOBACTERIAL THERAPY OF TUBERCULOSIS PATIENTS

There have been presented the standard daily doses of antituberculosis medications and the regimens of antimycobacterial, administered in different categories of tuberculosis patients. The optimal terms of first_line and second_line treatment and the term of main course of antimycobacterial therapy, depending on peculiarities of the disease, have been characterized.

MODERN ASPECTS OF PROVISION OF TUBERCULOSIS PATIENTS WITH ANTIMYCOBACTERIAL DRUGS

The basic approaches to a calculations of a need in antimicobacterial drugs, based on WHO recommendations and the author's experience, are presented. These calculations utilize 7_steps method. INTRAARTERIAL POLYCHEMOTHERAPY IN TREATMENT OF PATIENTS WITH CERVICAL CANCER

The modern trends in treatment of locally advanced forms of the cervical cancer, using intraarterial polychemotherapy (PCT), were presented. The therapeutic regimens and efficacy endpoints of treatment were characterized. The neoadjuvant intraarterial PCT increased a resectability of tumors in combined treatment, as well as improved the results of radiotherapy in patients with the cervical cancer.

SEQUENTIAL THERAPY WITH CIPROFLOXACIN IN MULTI-RESISTANT PULMONARY TUBERCULOSIS

In 46 patients with multiresistant pulmonary tuberculosis we studied the clinical effectiveness of 2 methods of administration of ciprofloxacin: sequential (i.v. followed by oral) and oral alone. In this category of patients a sequential combination therapy using ciprofloxacin was more effective (based on the analysis of major effectiveness variables - sputum abacillatioin, clinical recovery, pulmonary lesions involution) than conventional oral therapy with ciprofloxacin.

RESISTANCE OF MYCOBACTERIUM TUBERCULOSIS TO FLUOROQUINOLONES

In 6 months study the susceptibility of Mycobacterium tuberculosis (MBT) strains to fluoroquinolones, isolated from 77 patients with tuberculosis, was studied. Primary drug resistance to fluoroquinolones in these patients was revealed in 1 (1,2%) patient with newly detected tuberculosis. Secondary drug resistance to fluoroquinolones was registered in 25% of patients with chronic lung tuberculosis with drug resistance (5,0±0,4) to other antituberculous drugs after 8,3±1,6 months of polychemotherapy using fluoroquinolones.

EFFICACY OF ETHAMBUTOL FORMULATED AS RECTAL SUPPOSITORIES IN TREATMENT OF PULMONARY TUBERCULOSIS PATIENTS WITH CONCOMITANT GASTRO-INTESTINAL TRACT DISORDERS

The results of clinical study of the efficacy of ethambutol formulated as rectal suppositories (0,4 g, produced by "Monfarm", Ukraine) were presented. A clinical study was carried out in 102 patients with pulmonary tuberculosis with difficulties of oral administrations of medicines. Ethambutol in tablets (0,4 g produced by "Borschagivsky CPF", Ukraine) was chosen as a comparator. The obtained results allowed concluding, that both study medicines had been similarly effective. In fact, Ethambutol in suppositories had better tolerability profile in the patients with gastro_intestinal disorders.

PHARMACOTHERAPY OF MENTAL DISORDERS IN BREAST CANCER PATIENTS

34 women, suffering from breast cancer have been examined, using the clinical and psychological methods. The leading mental symptoms were predominantly anxious_depressive conditions. The high clinical efficacy of sertralin has been demonstrated.

COMPLEX TREATMENT OF PATIENTS WITH CONVEXITAL MENYNGIOMA OF BRAIN

We have conducted a complex clinical and hormonal examination and treatment of 64 patients with convexital menyngioma of brain (CMB) and hormonal disturbance syndrome (HD). The most often the HDs were associated with fronto_temporal localization of menyngioma. The type of HD depended on stage of the disease. In stages of subcompensation and mild decompensation the signs of hypotalamohypophysial hyperfunction (type I), while in stage of severe decompensation the inhibition of hypotalamo-hypophysial system (type II) were revealed. A clinical course of proximate postoperative period depended on grade of operation trauma and preoperative state of hypotalamo_hypophysial system. This period was characterized by deeper dysfunction of hypotalamohypophysial system. The rational tactics of complex treatment of patients with convexital menyngiomas of brain and HD syndrome has been worked out.

EFFICACY OF CIPROFLOXACIN IN A COMPLEX TR_ATMENT OF PATIENTS WITH ZONNE AND FLEXNER SHIGELLOSIS

The results of the study confirm high clinical efficacy of ciprofloxacin (250 mg twice daily per os during 5 days) for patients with acute Zonne and Flexner shigellosis. The therapy with ciprofloxacin in comparison with furazolidon has several advantages: a more rapid recovery of major clinical symptoms and higher clinical and bacteriological efficacy.

HIV INFECTION

The modern data on etiology, pathogenesis, clinical manifestations of HIV infection and AIDS, and peculiarities of HIV reproduction have been presented. The modern classification of HIV infection and antiretroviral medications for treatment and prophylaxis of this infection have been described. The authors have comprehensively characterized the mechanisms of antiretroviral effect, the indications and contraindications, the methods of administration of those compounds. A special attention have been paid to the chemoprophylaxis of HIV transmission from a mother to a child and antiretroviral therapy in pregnant women and children.

CLINICAL CLASSIFICATION, DIAGNOSTICS AND TREATMENT OF BRAIN MEMBRANES ANDCENTRAL NERVOUS SYSTEM TUBERCULOSIS

The methodical recommendations are dedicated to the topical problem of phthisiatry - tuberculosis of brain membranes and central nervous system (BMT and CNST). The definition, epidemiology in Ukraine, etiology and pathogenesis, clinical forms, therapeutic approaches and prognosis are presented.
The criteria of three grades of severity and differentiated therapy have been defined. A special emphasis has been put on treatment of BMT and CNST. The examples of diagnosis formulation are given.
