EFFICACY OF STANDARD ANTIBACTERIAL REGIMEN IN CASES OF FIRST-DIAGNOSED DESTRUCTIVE LUNG TUBERCULOSIS WITH BACTERIOEXCRETION

Summary. Data from publications and our observations with information about efficacy of standardized chemotherapy in cases of first-diagnosed destructive lung tuberculosis with excretion of Mycobacterium tuberculosis, in particular about controlled short-course therapy (DOTS) of this patients in Ukraine and Russia. We made conclusion that efficacy of standardized chemotherapy in cases of first-diagnosed destructive lung tuberculosis with excretion of Mycobacterium tuberculosis depend on intensity of chemotherapy, number and dimension of cavities. This factors will be taken into account when new standards of therapy will be created.

Summary. By the results of expert assessment of 9852 in-patients case reports prevalence of hospital-acquired pneumonia (HAP) in patients of multidivisional hospital is 0,4–2,5% between all patients and 12,1–37,5% between death cases (accordingly 0,7 and 18,5% average). Most significant risk-factors of HAP were identified: treatment of patients in ICU (in 82,1%), antibacterial therapy in forego period (in 76,8%), carrying of artificial pulmonary ventilation (in 71,4%), surgery (in 71,4%), declining years (in 60,7%), severe main disease with functional insufficiency of organs and systems (in 58,9%). Received dates will give in future possibility to control epidemic situation in each separate division owing to correction of arrangements to prophylactic of hospital-acquired infections.

DISTINCTIVE FEATURES OF PNEUMOCYSTIS CARINII INFECTION AT ADULT IMMUNOCOMPETENT PATIENTS WITH CHRONIC PULMONARY DISEASES     // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 39-44

Summary. To study factors influencing on presence of P. carinii in sputum, examination of 173 patients with exacerbation and in stage of beginning remission of chronic pulmonary diseases was performed with clinical, microbiological, cytological and immunological methods. It was determined the condition of temporary and lingering P. carinii persistence (colonization of low expiratory tract). There was the correlation between quantity of P. carinii in sputum and insufficiency of phagocytes. P. carinii persistence may reduce after traditional treatment of exacerbation of chronic pulmonary diseases.

SPECTRUM AND SUSCEPTIBILITY OF PATHOGENS TO ANTIBACTERIALS IN NON-HOSPITALIZED CASES OF MILD TO MODERATE COMMUNITY-ACQUIRED PNEUMONIA // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 45-50

Summary. In cases of mild to moderate community-acquired pneumonia, who treated without hospitalization, S. pneumoniae stay a major pathogen (more than in 50% of cases) and keep high level of susceptibility to b-lactams and macrolids (more than 90%). In this casesvariable weight of atypical pathogens (M. pneumoniae and C. pneumoniae) was more than 20%.

THE DRUG ALLERGY IN CHILDREN WITH INFECTING BY MYCOBACTERIUM TUBERCULOSIS // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 51-54

Summary. Some aspects of drug allergy in children infecting by mycobacterium tuberculosis are given. The rate of drug allergy in these cases makes up 2,7% and tends to further increase. A list of the drugs more frequently causing allergic reactions was cite. In antituberculosis institutions it seems necessary to organize proper prophylaxis, diagnosis and treatment of drug allergies that would meet modern requirements.

LUNG TUBERCULOSIS AND WITH MORE THEN ONE CAVERN DEPENDING ON SIZE OF CAVERN AND INTENSIVE OF CEMOTHERAPY // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 55-60

Summary. In this article dates about the treating efficacy of patients with newly detected sputum positive lung tuberculosis with more then one cavern depending on size of cavern and intensive of chemotherapy were cited: with using 4 antituberculous drugs and with caverns less 4 cm (first group) or with using 5 antituberculous drugs and with caverns less 4 cm (second group); with using 4 antituberculous drugs and with caverns 4 and more cm (third group) or with using 5 antituberculous drugs and with caverns 4 and more cm (forth group). It has been established that: sputum conversion in 93,2±3,3% or in 100% patients of the first or second groups, in 59,1±10,7% or in 88,2±8,1%% patients of the third or forth groups. Caverns healing on the lung in 69,5±5,9% or in 86,7±6,2% patients of the first or second groups; in 22,7±9,1% and in 29,4±11,4% patients of the third or forth groups.

THE TREATING EFFICACY OF PATIENTS WITH NEWLY DETECTED SPUTUM POSITIVE DETERMINATION OF THE OPTIMUM RATIO OF ISONIAZID AND VITAMIN B6 FOR TREATMENT OF GUINEA-PIGS IN CONDITIONS OF EXPERIMENTAL TUBERCULOSIS // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 61-64

Summary. With the help of specially planned factorial experiment — treatment of guinea-pigs with an experimental tuberculosis by various dozes of isoniazid and vitamin B6, the mathematical model which describes this process is constructed. Severity of tuberculosis value by index of affection of internal organs of guinea-pigs. As factors which influence on index of affection were used: isoniazid as a first factor and vitamin В6 as a the second factor. Each factor was studied at three levels of doses. On the basis of the constructed mathematical model the optimum ratio between dozes of isoniazid and vitamin В6 which average 10 and 26 mg/kg has been determined.

The results of conducted study evidenced that application of levofloxacin in dose 250 mg per day durin 10 days in complex treatment of patients with relapse of chronic pyelonephritis increases the effectiveness of treatment, has no adverse reactions and deteriorates the antibioticoresistance of ethiologic agents development.

Summary. 56 patients with squamous cell cervical carcinoma were treated by chemoradiotherapy (CRT) or radiotherapy (RT) alone in the department of radiotherapy Institute of oncology AMS of Ukraine. The RT regimen were the similar at all cases. 26 women were treated by radiotherapy with lymphotropic аdministration of methotrexate and fluoruoracil infusions. These data showed good patient tolerance and encouraging response to RT combined with chemotherapy.

Summary. The efficacy and safety profile of full-dose capecitabine were provided during the treatment of 46 patients with different type of disseminated adenocarcinomas — breast cancer, colorectal cancer, gastric cancer, renal cancer. Our results demonstrated high efficacy and low toxicity of capecitabine as monotherapy for lung and hepatic metastases.

EFFICACY OF ADJUVANT TREATMENT WITH COMBINATION OF UKRAIN (NSC-631570) AND GEMCITABIN IN CASES OF PANCREAS CANCER // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 83-86

Resume. The purpose of this study was to evaluate the survival in 23 pancreatic head cancer patients after curative pancreaticoduodenal resection treated by combination of Ukrain and gemcitabin. The 2 year OS and PFS were 60% and 20% respectively in 15 patients (main group) treated by combination of Ukrain and gemcitabin. In 8 patients of untreated control group there were no 2-year survivors. The difference in OS was statistically significant. The median survival was 32 months in main group vs 10 months in control. Application of the combination of Ukrain and gemcitabin is effective in adjuvant treatment of patients with pancreas cancer.

USE OF METHOTREXATE IN HIGH DOSES FOR THE TREATMENT OF CHILDREN WITH MALIGNANT BONE NEOPLASMS // Ukr. J. Chemotherapy.- 2005.- N1-2.- P. 87-89

Summary. 38 children aged 8–16 years received methotrexate in a dose of 12 g/m2 in complex therapy of malignant bone neoplasms. According to the data of biochemical analyzer TDX-FLX, by means of immunofluorescence method, the observation of preparation level in blood serum and the prescription of sufficient doses of calcium folinate were daily performed. There were no cases of death. The complications in form of skin reactions, hepatotoxicity lst–4th degree, delay of methotrexate elimination, moderately pronounced gastrointestinal and myelosuppressive effects were observed. The obtained results suggest a good tolerance of the preparation and its safety under condition of apparatus monitoring of its elimination, ensuring of a sufficient diuresis level, urine alkalization and prescription of calcium folinat in sufficient dose and for sufficient time.

Summary. Ondansetron (Osetron, Dr. Reddy's) was used for the treatment of nausea and vomiting during 56 blocks of highly emetogenic combine polychemotherapy in 14 patients with high risk of postmolar trophoblastic tumors. Acute and delayed vomiting wasn't registered. Acute nausea occurred in 7,1% of cases, delayed nausea — in 14,3% cases.

Modern approachments to treatment of patients with HAP eludicated in guideline. Placed high emphasis on rational using of antibacterials. Expanded criteria of efficacy of treatment and scheme of treatment of patients with HAP depending on peculiarities of its course, that allow to unify requirements to medical care in case of HAP and to raise efficacy of their treatment in multifield hospital.
