A situation with tuberculosis in Ukraine have been analyzed. There have been worked out a program of financing and conducting of antituberculous measures under modern social-economic conditions. Clinical and diagnostic peculiarities of the tuberculous meningoencephalitis course in adults and children

The authors analyzed the questionnaires filled by phthisiologists of all regions of Ukraine in order to reveal the peculiarities of the tuberculous meningoencephalitis course. The most numerous cases were revealed in 1998 year in Zaporizhska region - 11 (14,7%), Donetska, Zhytomirska and Lvivska regions - 7 (9,3%). The main origin of the meningoencephalitis was pulmonary tuberculosis (61,4%); less often - extrapulmonary tuberculosis (21,1%); in 17,5% patients the meningoencephalitis had been diagnosed without any other changes attributed to tuberculosis. The clinical and diagnostic peculiarities of modern course of meningoencephalitis and therapeutic options have been described.

Cohort studies as part of clinical monitoring in patients with lung tuberculosis and COPD

Main principles of cohort studies and its practical implementationon in patients with chronic lung diseases (lung tuberculosis and COPD) were described on the basis of normative regulations and literature data. Advantage of cohort studies has been shown in both phtisiatric and pulmonologic patients. FEV1 and quality of life assessment were disease-specific determinants in monitoring of patients with COPD. Expert computer systems design has also been considered as a powerful tool for clinical monitoring in prospective studies.

The evaluation of risk of tuberculosis in children of IV and VI-A, B dispensary follow-up groups

On the basis of Wald method there was composed the table for evaluation of the tuberculosis risk in children of IV and VI-A, B dispensary follow-up groups. This prognostic table made it possible to reveal the persons with high risk of the disease for an application of adequate preventive measures.

Differential diagnostics of tuberculous and carcinomatic pleurisies by means of blood laser correlative spectroscopy (LCS)

The plasma subfractional structure of 100 patients with exudative pleurisy was studied. In 50 patients a tuberculous pleurisy was diagnosed; carcinomatic pleurisy - in another 50 patients. Using LCS the changes of homeostasis were discovered in plasma of patients with tuberculosis and cancer. This allowed to differentiate reliably these diseases in 68-72 % of cases.

Residual changes in spontaneously cured servicemen in regular military service with lung tuberculosis and tuberculosis of intrathoracic lymphatic nodes and pleura

There were analyzed 1090 cases of fluorographically examined servicemen; 162 patients, operated for pulmonary tuberculosis, tuberculosis of intrathoracic lymphatic nodes and pleura, and residual changes in lungs after antimycobacterial therapy; 35 cases of autopsy of patients who died from nontuberculous diseases. The frequency of residual changes after the spontaneously cured tuberculosis in the servicemen was 4,0 %. In this group 34,09% of cases could be activated during the first year of service. Chemoprophylaxis prevents a development of tuberculosis. Screening for residual changes after spontaneously cured tuberculosis should be based on examination of both latter and archive fluorograms. We proposed the classification of residual metatuberculous lung changes (including spontaneously cured tuberculosis), which reflected etiopathogenesis and involution of the disease. The authors do not attribute so called "sanitated cavity" to residual changes after cured tuberculosis. The patients with sanitated cavities should be operated.

The state of energy metabolism and oxidant-antioxidant system and its correction with millimeter range electromagnetic waves in tuberculosis patients, undergoing chemotherapy

We have studied the influence of millimeter range electromagnetic waves on adenosinetriphosphatase, aerobic and anaerobic glucolysis enzymes activity, as well as on pentosophosphatic cycle and oxidant-antioxidant system in tuberculosis patients, undergoing antituberculous chemotherapy. We have established that application of electromagnetic wave therapy on the background of antituberculosis chemotherapy caused additional therapeutic effect: the normalization of energetic and anabolic metabolism was revealed.

Efficacy of low-frequency ultrasound therapy in complex treatment of newly diagnosed cases of pulmonary tuberculosis and its influence on lipids peroxydation system and level of middle mass molecules

The study of clinical efficacy of low-frequency ultrasound therapy in complex treatment of patients with lung tuberculosis showed that the application of low frequency ultrasound promoted a quicker resolution of infiltration, caverns healing in the lung tissue and abacillation. At the same time the method had microdestructive influence which manifested in insignificant intensification of lipids peroxydation, some decrease of antiradical protection and increase of internal intoxication level.

Pharmacotherapy of chronic obstructive pulmonary diseases

In the article the clinical characteristics of 6 basic groups of medicinal preparations for treatment of chronic obstructive pulmonary diseases of lung is given according to the international agreement. The practical guidelines on application of selective (2-agonists, cholinolytics, non-steroid anti-inflammatory drugs, corticosteroids and mucolytics are presented. The new aspects of clinical application of a number of well known medicinal preparations are reflected.

Peculiarities of pneumonia in diabetes mellitus patients

There were studied the clinical peculiarities of pneumonia in 185 diabetes mellitus (DM) patients. Both diseases worsened clinical course of each other, leading to more severe pneumonia symptoms and diabetes decompensation. The inflammation in lung tissues was more extended and resistant, causing prolonged, sometimes relapsing course of pneumonia, and evoking numerous complications.

The role of bronchial obstruction in pathogenes1s of heart failure in patients with cor pulmonale

Considering a role of extracardial factors in pathogenesis of circulatory failure in cor pulmonale patients, we have studied an interrelation between bronchial passability disturbances (known cause of increased intrathoracic pressure) and both pulmonary and systemic circulation indices in group of patients with chronic bronchitis in combination with heart failure. It has been proved that increased intrathoracic pressure led to manifestation of right heart failure.

Medical and economical substantiation of optimization of medical and diagnostic process in patients with pneumonia

The article deals with economic aspects of pneumonia patients management. The group of patients that should be treated as out-patients (64%) has been outlined. The results of the study have demonstrated, that under market conditions an optimization of treatment would allow to use more rationally the resources of Healthcare system.

The in vitro study of antiaggregative properties of medicine Tekom-A

There have been compared the influence of Tekom (45% of omega-3 polyunsaturated fatty acids), acetylsalicylic acid and Tekom-A (combination of Tekom with acetylsalicylic acid) on platelets aggregation. It have been established that Tekom and acetylsalicylic acid in vitro possessed a prominent antiaggregative effect. Tekom-A, comparing with Tecom and acetylsalicylic acid, was significantly more effective concerning an inhibition of platelets aggregation. The results obtained necessitate further clinical studies on antiaggregative effects of new medicine Tekom-A.

Role of bronchoconstriction in development of impaired pulmonary function in miners

Pulmonary function tests were performed in 55 healthy miners after a working day. The initial impairments characterized by both restrictive and obstructive changes were revealed. The test with salbutomol has not revealed a latent bronchoconstriction.

Inhaled glucocorticosteroids in regular treatment of children with asthma

The aim of the study was to determine efficacy and safety of inhaled corticosteroids in children. 80 children aged 4-15 years with moderate and severe asthma were examined. Proposed method of application of inhaled corticosteroids in treatment of children with asthma was effective in almost 90% of patients.

Immunological and radioimmunological diagnostics of the lung cancer revealed at prophylactic fluorographic examination

We conducted a complex clinical, X-ray, thermographical, immunological and radioimmunological examinations in 56 patients with lung cancer revealed during preventive fluorographic examination. Generally accepted clinical and laboratory examinations gave preliminary information only in 26,8% cases. 73,2% of patients required complex reexamination using the modern instrumental and laboratory methods. The RIA evaluation of carcinoembryonic antigen level in blood serum appeared to be the most valuable for detection of the malignancies.

Bronchial mucous membrane changes in Chernobyl disaster consequences liquidators revealed by special pathohistological method

The results of the study revealed a destructive-dystrophic and necrotic changes in bronchial mucosa of Chernobyl disaster consequences liquidators. Those lesions were concentrated around specific pseudoamiloid grains. Pseudoamiloid grains arose under the influence of ionizing radiation. The changes described could be found using special pathohistological method.

Final pneumonectomy in patients with respiratory diseases

The authors analyzed the results of final pneumonectomy in 136 patients of 4 groups: various postoperative complications (13), recurrent chronic purulent lung diseases (65), recurrent tuberculosis of the lung with chronic postoperative pleural empyema (48), and recurrent tumors in the resected lungs (10). Despite the successes of thoracic surgery, such operations are still technically difficult, traumatic, and associated with a high risk of serious complications (36,8%) and mortality (15,4%). It was proved that application of the differential technique accounted for higher clinical effectiveness of the operation.

Efficacy of endolymphatic injection of medicines in patients with respiratory diseases

Endolymphatic injection of medicines is highly effective method of treatment of patients with extended progressive forms of destructive pulmonary tuberculosis and nonspecific purulent lung and pleura diseases. This method is strongly recommended in cases of multiple drug resistant infections. Endolymphatic injection of medicines in preoperative period led to better outcomes of operation, even in those cases where surgical intervention had been contraindicated. Endolymphatic injection of medicines in patients with purulent lung diseases increased treatment effectiveness and decreased mortality.

The ways of increase of infectious tuberculosis patients detection within Healthcare network

The approbation of the program "The revelation of infectious tuberculosis patients in the institutions of Healthcare network" was performed in 7 districts and 3 cities of Zaporizhska region. The method of smear microscopy using Ziehl-Neelsen staining was implemented. The rate of detection of TB case based on microscopy was 4 times higher than those using fluorography (2,5 and 0,7 cases per 1000 examined patients, respectively). 28% of all infectious TB cases and 14,2% among new cases were smear positive. The rate of smear examination was 2,5 per 1 patient. 1-st smear was positive in 67%, 2-nd - in 22%, 3-d - in 11% of cases. The price of 1 case-finding by means of smear microscopy was 7,5 times cheaper than using fluorography method.

The problem of respiratory Healthcare service informatization in Ukraine

The current state and prospects of respiratory Healthcare service informatization in Ukraine were analyzed. The problem of informatization is in a very initial stage of its solution. At present state leading pulmonary practitioners could start building up a service informatization conception, subject sphere integral model, common terminological system and subject thesauruses, medical information formalization and standardization of documents. It will help to create some normative and methodic documents and formulate technical tasks for realization of informatization of respiratory Healthcare service. The main methodological approaches have been stated.
