Monitoring of the tuberculosis incidence and mortality in ukraine in people of different age and sex groups (analysis for 1990-2000)

There have been carried out a monitoring of an epidemic situation on tuberculosis in Ukraine during last 11 years (1990-2000). There was estimated a quantity of cases in different age and sex groups in structure of the total tuberculosis morbidity. The growth of total tuberculosis incidence, the incidence of pulmonary tuberculosis, rate of smear-positive tuberculosis and tuberculosis mortality was revealed. 71-73 % of all tuberculosis cases were new cases in men. This parameter was 2,3-2,7 times higher in men than in women. More often new cases of a tuberculosis were registered in people of 0-59 years age group. In the group of people of 60 years and older a stable tuberculosis incidence and mortality were observed. Thus, it is necessary to create a computer network for prospective control of situation with tuberculosis in Ukraine.

Assessment of the activity of phthisiopediatric service in ukraine and the ways of improvement of the effectiveness of pediatric antituberculosis care

Maximum working productivity of respiratory system in healthy people of different age groups

Maximum productivity of respiratory system was evaluated in 192 healthy men and 106 women aged 18-95 years. Ramp exercise test with 12,5 W/min increase of power was used. Age relative changes of respiratory system productivity at a level of maximum oxygen consumption were studied. The ventilation and gas exchange were determined by means of an automated gas analyzer "Oxycon-4" (Mijnhardt, Netherlands). A distinct decrease in maximum physical performance, maximum oxygen consumption, maximum minute ventilation, oxygen pulse was observed with aging, both in men and in women. An increase of the minimum ventilation, oxygen-equivalent and carbonic acid-equivalent ventilation was marked with aging. The decrease of maximum pulmonary ventilation had linear dependence with aging, whereas the value of a minimum oxygen-equivalent ventilation and carbonic acid-equivalent ventilation changes - only in older age. The effectiveness of respiratory system deteriorated with aging. The possible mechanisms of age-relative changes of maximum pulmonary productivity were reviewed.

Pathogenesis and methods of treatment of congestive heart failure in chronic obstructive bronchitis patients

The study was based on comparative analysis of lung ventilatory function and hemodynamics indices in patients with chronic obstructive bronchitis (COB) and coronary artery disease (CAD), presenting initial signs of congestive heart failure. It was established that in COB patients some extracardial factors (e.g. increased intrathoracic pressure, leading to reduced venous return and extracardial blood deposition) played an important role in development of congestive heart failure. An administration of medicines, increasing inotropic function of heart, is indicated in COB patients only with NYHA III heart failure, caused by significant right heart dilation and impaired heart contractility.

Cardiac arrhythmias documented by 24-hour electrocardiographic monitoring in patients with chronic obstructive bronchitis

To study the frequency and character of cardiac arrhythmias, 66 patients with chronic obstructive bronchitis (COB) without cardiovascular diseases were examined. The physical examination, standard ECG, spirometry, pulse oxymetry and 24-hour out-patient electrocardiographic monitoring were performed. The most frequent arrhythmias documented by Holter monitoring were supraventricular extrasystoles. The frequency and character of cardiac arrhythmias increased depending on a stage of COB. Episodes of ST-segment depression in patients with COB were registered at increase of heart rate over 100 bpm. We considered these changes to be connected with arterial hypoxemia, which developed because of alveolar hypoventilation in bronchial obstruction.

Structural and functional peculiarities of upper respiratory tract in patients with chronic obstructive bronchitis and bronchial asthma, combined with sleep apnea syndrome

We enrolled 50 patients with chronic obstructive bronchitis (FEV1 (66,7±5,4) %) and asthma (FEV1 (72,5±5,32) %), aged 22-74 years (mean age 54±1,9 years), presenting the most signi-ficant signs of sleep apnea syndrome (SSA): pauses during the sleep, snoring, sleepiness during the day, short falling asleep du-ring the routine work or in the transport.
The evaluation of structural and functional peculiarities of upper respiratory tract allowed us o concluding, that in patients with "overlap syndrome" a snoring and obstructive apnea could appear as a result of narrowing of respiratory tract on the different levels, mainly in the nasopharynx.
As a result of our study the indications for polysomnographia for confirmation of SSA in chronic obstructive bronchitis and asthma patients have been worked out.

Stage dependent evaluation of quality of life in patients with chronic obstructive bronchitis

The quality of life (QL) was assessed using the St. George's Respiratory Questionnaire (SGRQ, Great Britain) in 117 patients with chronic obstructive bronchitis (COB). The results of the study showed that the "Total score of QL" and its components, such as "Symptoms", "Activity", "Impacts" of the SGRQ, reflected the health disturbances, which greatly depended on stage of the disease. The limits of changes in the "Total score of QL" and its components were determined for each stage of COB.

Quality of life in patients with exacerbation of asthma, treated with inhaled corticosteroids in high doses

A quality of life in the patients with exacerbation of asthma, treated with corticosteroids budesonide and fluticasone propionate and systemic corticosteroid dexametasone in 90 patients was assessed. The drugs significantly improved quality of life scores. Fluticasone propionate in comparison with other drugs showed the statistically valid improvement in quality of life, especially in emotional sphere. There was no difference between dexametasone and budesonide in improvement of quality of life.

Pneumocystosis in adults with chronic nonspecific pulmonary diseases

In 84 immunocompetent adults with chronic bronchitis and asthma we have studied an influence of Pneumocystis carinii persistence in sputum on clinical course of underlying disease. We established that 60 % of examined patients were infected with this pathogen. The presence of Pneumocystis carinii worsened clinical course of disease. Pneumocystosis infection often occurred under poor living conditions (professional exposures, sensitization, cold weather).

Dynamics of primary disability in asthma patients among the inhabitants of vinnitsa area

The study of primary asthma disability indices among the inhabitants of Vinnitsa area was carried out. It was established that the reduction of primary disability prevalence occurred in the group of patients 60 years and older. In young patients the physical disability prevalence has increased. The relative quantity of disabled persons with asthma among all disabled patients with pulmonary diseases has also increased (29 % in 1993, 50,6 % in 1998). Among the men of 28 years old and younger the index of disability increased in 3 times; among the women of the same age group - in 1,4 times.

Application of laser therapy in treatment of asthma in children

The results of follow-up of 65 children with asthma, who underwent a procedure of low-intensive laser accupuncture or low-intensive transcutaneous laser radiation, have been presented. Both methods could be used as additional therapeutic procedures in treatment of asthma in children.

The efficacy of licorice root infusion in treatment of patients with asthma, complicated by gastrointestinal diseases

To evaluate the efficacy of licorice root infusion we examined 67 patients with asthma, complicated by gastrointestinal diseases (38 patients had concomitant chronic gastritis, 15 - erosive gastroduodenitis, 14 - ulcer). All patients received complex therapy, which included corticosteroids and b2-agonists. In 51 cases we additionally administered a licorice root infusion. The results of treatment have demonstrated significant clinical and endoscopicaly confirmed improvement. This study allows to recommend an administration of licorice root infusion in treatment of patients with asthma, complicated by gastrointestinal diseases.

A state of some detoxicative systems in experimental acute bronchitis and administration of tekom

On experimental model there have been demonstrated a failure of glutation-dependent system of detoxication of erythrocytes and cytochrome P-450 in acute bronchitis. It was proved that medicine Tekom possessed a corrective effect regarding the studied systems.

The hormone-dependent fibrinolytic activity of blood leukocytes in patients with chronic lung abscess, bronchiectasis disease, cystic lung disease, and lung cyst

It was established that autological leukocytes reduced the lysis of the euglobulin clot in patients with chronic nonspecific lung diseases during the preoperative period. A fibrinolytic activity of the leukocytes was restored under the influence of pre-incubation with thymalin and chorionic gonadotropin. These facts were estimated as the pathophysiological background of the placental hormone administration for correction of misbalance of leukocyte fibrinolytic activity in this group of patients.

Clinical role of L-transformed mycobacteria in patients with newly diagnosed pulmonary tuberculosis

Microbiological, immunological and biochemical examination was conducted in 159 patients with newly diagnosed destructive pulmonary tuberculosis. A prevalence of L-forms of mycobacterium tuberculosis was 44,2 % in all microbiological isolates. At the same time L-forms only were isolated solely in 10,7 % cases, and in combination with bacterial forms - in 18,2 % of cases.

Bacteriostatic blood activity against the strains of m. Tuberculosis, isolated from pulmonary tuberculosis patients, and laboratory strain h37rv during the course of polychemotherapy

The aim of the study was to evaluate bacteriostatic blood activity (BBA) against M. tuberculosis strains, isolated from patients with pulmonary destructive tuberculosis, and against laboratory strain H37Rv during the course of intensive polychemotherapy. BBA was studied in 46 patients with newly diagnosed pulmonary destructive tuberculosis against the strains of M. tuberculosis, isolated from these patients, and in 56 patients with similar process against the strain H37Rv. All patients received polychemotherapy regimens, consisted of 5-6 basic antituberculosis drugs, with daily administration of 5 drugs. Maximum BBA levels reached high values (bacterial growth inhibited in blood dilutions 1:32-1:512) in 100% of cases, despite of 23,9% of newly diagnosed patients with pulmonary destructive tuberculosis having had primary antituberculosis drug resistance. In 89,1% of cases we determined only high or high and moderate levels of BBA during 24 hours against M. tuberculosis strains, isolated from these patients. BBA levels against laboratory strain H37Rv were much the same or slightly higher than those mentioned above.

The results of treatment of destructive pulmonary tuberculosis using flurenizid

The clinical effectiveness of the flurenizid in the complex chemotherapy of the patients with newly detected destructive pulmonary tuberculosis was studied. It was proved, that complex treatment of these patients using flurenizid promoted high clinical effect without development of toxic or toxic-allergic reactions. The results of the treatment (isoniazid was substituted by flurenizid in cases of an intolerance to isoniazid or parenteral isoniazid was alternated with flurenizid in tablets 0,15 g) have been described. The obtained data enabled to recommend flurenizid as a high-performance antituberculous drug in daily or intermittently administered regimens of chemotherapy.
